The complexities of aging or coping with a disability can be overwhelming. Often our clients come to us with more questions than answers. What is really happening with my loved one’s health? Kari Klatt is a certified Aging Life Care Manager, practicing care management with disabled and older adults since the late 1970s. She began her career as a Registered Nurse and went on to obtain her Master’s in Social Work. Kari has also held leadership positions with Stowell Associates and Froedtert & the Medical College. Kari has experience in navigating health care systems, new diagnoses, life planning, relocation, health care benefits and end of life care. When she is not working or caregiving, Kari enjoys hiking with her dog Cooper, Pilates and cycling.
Wenda Krupp’s nursing career spans nearly 40 years, working with geriatric patients in cardiology and critical care. For the last decade, she has served in positions practicing and leading care management. Her approach to care management is based on active listening, hearing the client’s story and understanding what matters most to them. Allowing her to quickly develop trusting relationships with her clients and assisting them to meet their goals. Her areas of experience and interest are medical care coordination and advocacy, assisting seniors and their families to access and navigate the complexities of healthcare, family caregiver support, and advance care planning. In her spare time, Wenda loves spending time with her husband and three adult children, gardening, singing and exploring Wisconsin.
Jennifer Nowak is a Master’s prepared Social Worker who has been working as a Certified Social Worker Care Manager for over ten years. Jennifer has a long history of working with individuals with dementia and their families. As a strong advocate, Jennifer works with clients and their families to find needed resources to remain in their homes. Jennifer was previously an Administrator of a dementia specific day center for over eleven years. One of her biggest achievements was developing and starting Wisconsin’s first Early Memory Loss program in 2005 which became a model for other such programs throughout the state. In her free time, she enjoys gardening, boating and being outdoors.
Miriam Oliensis-Torres, is a licensed clinical social worker and certified care manager, with extensive experience assisting older adults, people living with dementia their families, and other service providers. Miriam has been in leadership roles in two Milwaukee area care management agencies, Geriatric Support Associates, and Stowell Associates for more than 30 years. She has served on local, regional, and national boards, and is a regular presenter at community and professional educational events. Her practice centers on gaining an understanding of the client’s situation, assisting them to understand their options, and guiding them through the process of making and implementing decisions about how to best address current and future concerns. When not working, Miriam spends time with her family (husband, three adult children, and three grandchildren), enjoys Wisconsin summers, travels to visit friends and family, and is an active volunteer in her community.
Jennifer Teglia’s healthcare career has included hospital, home hospice and palliative care nursing. She became a Professional Care Manager in 2016 and has enjoyed working in this capacity ever since. Her areas of interest include medical care coordination and advocacy, discharge planning, home safety, and in-home care. She can assist with referrals for palliative or hospice care and home therapies. Jennifer is a member of the Aging Life Care Association (ALCA) and the National Association of Certified Care Managers (NACCM). She is dedicated to assisting her clients and their families as they navigate the complex world of healthcare and the journey of aging. Jennifer loves spending time with her husband, family and friends. She also enjoys hiking, biking, exploring and traveling.
Susan Wack is a Certified Aging Life Care Professional who has worked as a Social Worker for over 25 years in the fields of developmental disabilities, long term care, home health, medical social work, and aging services. Susan has trained in domestic relations mediation and has provided eldercare mediation training to attorneys and social workers in the state of Indiana. She has a bachelor’s and master’s degree in social work and enjoys using her skills to assist older adults, individuals with disabilities and their families in making informed decisions while navigating the complex fields of social services and health care. In her spare time, she enjoys reading, being in nature, and spending time with family and friends.
