Clients may be unaware of when and how to utilize beneficial services such as therapy, home health, palliative and hospice care. Our care managers understand these services and will provide specific recommendations. In addition, we understand the complexities of family dynamics that can arise when making medical decisions. Short & Long Term Planning

Planning for the future can be a daunting task that many put off.  Our Care Managers use empathy and active listening to understand areas of well-being related to health, family, finances, legal and housing. Similarly, aging well is more likely with the help of an experienced Care Manager and a personalized plan.

Legal Decision Maker

As an aging adult, it is natural to want reassurance that your goals, preferences and beliefs will be respected even if you are unable to communicate them yourself. Legal documents naming an individual to act in this role are required in Wisconsin. While some aging adults have a family member or friend who can act in this role, others may not know who to turn to.

as health care and/or
fiscal agents.  Our care managers prioritize getting to know you, your goals/beliefs, and your unique needs. This enables us to effectively advocate for you when you are no longer able to for yourself. You can rest assured that our protocols maintain professional ethics and standards of practice, providing you peace of mind.

Guidance Through Transitions

Our Care Managers are a bit like the “wind beneath your wings”, guiding you through life’s transitions whether it is after a hospital visit, short-term rehabilitation stay, or a relocation from your home. We provide decision-making support and a thorough exploration of appropriate referrals to ensure a seamless transition. From coordinating the move to guaranteeing a smooth settling-in process, we take care of every aspect.

Medication Management

Our care managers will create and maintain a comprehensive medication list, monitor for appropriate use and handle refills. Our care managers will also communicate with physicians regarding medication changes, observations, questions or concerns.

Memory Support

If your loved one is showing signs of cognitive issues (thinking, memory, judgement, etc.) or already has a related diagnosis (Alzheimer’s, dementia, Lewy Body, etc.), it is understandable to feel unprepared for what is ahead. Our team, highly trained in dementia care, is here to help. We truly believe education is the best tool to help family members cope
with their loved one’s changes.

By learning why your loved one is acting the way he/she does, as well as how to communicate and respond to him/her, we can find opportunities for meaningful engagement and even enjoyment. Our Care Managers will walk along this seemingly daunting path with you, providing recommendations related to care, solutions associated with “behaviors”, caregiver burnout prevention, general support, and more.
